Why did Health and Welfare decide to contract its Res Hab Program Coordination services to only one agency?
In a letter to CFH Providers, dated 6/10/11, the Idaho Dept. of Health & Welfare’s Medicaid Division explained: “Over the past few years, Idaho Legislation has directed the Department to enter into selective contracts for services. This direction was first provided to the Department in 2006, subsequently in House Bill 701 in 2010, and finally by direction contained in House Bill 260 in 2011. In compliance with this legislative direction, the Department would like to announce that a contract has been awarded to Community Partnerships of Idaho, Inc. for ‘Residential Habilitation Program Coordination for Certified Family Home Providers’. This contract was executed on June 6, 2011, and will be implemented on August 5, 2011.”

When does the change in Program Coordination for Certified Family Home providers go into effect?
Community Partnerships of Idaho, Inc. (CPI) has 60 days to complete our Implementation Plan. The contract will go into effect on August 5, 2011 with CPI beginning its role of Program Coordination for the state.

Will there be any interruption of services between now and August 5, 2011?
There will not be any interruption in any services provided to the CFH participant living in your home as a result of implementing this contract.

What services will Community Partnerships of Idaho provide to CFH Providers?
Services which will be provided as part of the Idaho Department of Health & Welfare’s contract with Community Partnerships of Idaho include (but are not limited to) the following:
• CFH Orientation and Training
• Provider Skills Training
• Training on completing CFH provider status reviews
• Access to training modules and community resources
• Provide review of implementation plans quarterly
• Assistance with Molina billing as requested
• Face-to-face meetings one time per year
• Training at yearly face-to-face meeting related to implementation plans
• Follow-up phone calls to providers as needed to discuss implementation program plan changes.

Who is the new Program Coordinator that I will be working with? When will I find out who the new person is? When will we meet him/her?
Community Partnerships of Idaho, Inc posted the Program Coordinator positions beginning June 10 and closing July 1. It is our hope that current Program Coordinators will be interested in continuing to provide this service to Certified Family Home providers and their participants and will apply for one of the positions. All Program Coordinators will be hired and trained by the contract start date of August 5, 2011.
Meeting Program Coordinators -
- On August 5th, the Program Coordinators will begin contacting their CFH providers.
- During the first week of August, we will send all CFH providers a letter that informs you of who your Program Coordinator is, how to reach them and when you can expect to hear from them.
- In August and September, we will be hosting local meetings with CFH providers, participants, family members and service coordinators to introduce our Program Coordinators and answer questions about the contract.

How can I get help if I it need before August 5?
Your current Program Coordinator should continue to be available to assist you until CPI takes over this program on August 5, 2011.

The Program Coordination agency that I am currently with does my billing for me. Who will assist me with my billing when the contract starts?
You may submit your own billing to Molina. Please reference the letter you recently received from the CFH unit that included a detailed 6 page step by step instruction guiding you on billing through Molina. You can also access this information on the state’s CFH website: www.cfh.dhw.idaho.gov.

Will my Targeted Service Coordinator be changing?
No. The statewide contract is solely for Program Coordination for Certified Family Homes and does not affect the Targeted Service Coordinator that is working with participant(s) in your home.

If I have concerns about any case management or crisis need, who should I contact for assistance?
The recommended contact for assistance is the participant’s service coordinator. The service coordinator should be contacted:
• When it is necessary to change a participant’s individual service plan (ISP) or specific goals or actions because of the participant’s medical, psychiatric, social, educational or other service needs.
• When the participant needs help linking with activities related to their medical, psychiatric, social, educational or other service needs.
• When monitoring or follow up service coordination activities are necessary due to a change in the status of the participant, making it necessary to adjust the plan and arrange additional services.
• When a participant is in crisis and needs to access community resources in order to resolve a crisis.

How will Community Partnerships of Idaho help with my Provider Status Reviews?
- CPI Program Coordinators will assist you with completing your participant’s 6 month and annual Provider Status Reviews.
- CFH providers will have the option of utilizing our website (www.CPIResHab.com). You will be able to input your participant’s data online and their monthly progress will automatically be computed and downloaded onto the Provider Status Review forms. You will then be responsible for inputting a progress summary statement and then printing a copy to send to the TSC.
- If you do not utilize the website, Program Coordinators will assist you to complete it by hand. More information on this support will be provided when your Program Coordinator calls you in August.

How will Community Partnerships of Idaho obtain participant files?
- To assist in a smooth transition, CPI has requested a copy of your participant’s current ISP from his/her TSC. In addition, we have requested copies of your participant’s current Residential Habilitation Plan, Program Implementation Plans, and Provider Status Review from your Residential Habilitation Agency.
- If we are not able to receive this information from these entities, we will need your assistance to get copies of these documents from your home files. If this is the case, we will let you know in late July.

If I choose not to bill Molina directly, can I access a billing agency to help with my billing?
YES. There are many agencies that offer billing services for CFH providers.
